Additional Information: Formal Name Change

Minor name changes

If you are applying for a minor name change (incl. addition of a middle name, spelling correction or a change of name order) the following documents will be accepted:

● Australian Birth Certificate    ● Australian Passport    ● Other Passport and Australian Visa Label/ Letter  

 ● Australian Driver Licence

Full name changes

If you are applying for a full name change after legally changing your name, the following documents will be accepted:

● Australian Change of Name Certificate   ● An amended Australian state or territory birth certificate

Name change after marriage or divorce

If you require a name change after marriage or divorce, the following documents will be accepted:

● Marriage Certificate    ● Divorce Certificate

Graduating Students

If you will be graduating, your request to change/correct your name must be received no later than the deadline indicated in the conferral email from the Graduations Office. Change requests received after this deadline will not be reflected on your graduations documents.
